The undersigned hereby assigns this claim to Optical Recovery for the purpose of collection. The undersigned further authorizes Optical Recovery to forward this claim to an attorney with prior approval if necessary to effect payment. The undersigned also agrees that Optical Recovery may deduct commission on claims paid directly to Creditor, and it further agrees that Optical Recovery is entitled to all commissions for all money recovered whether paid to Optical Recovery or directly to Creditor and authorizes Optical Recovery to endorse negotiable instruments received in payment of claims. In case suite or action is instituted to collect such commissions on these assignments, the undersigned promises to pay all collection costs such additional sums as the court may adjudge reasonable such as court costs, attorney fees, etc.
